Most specialty practices are paying to win the same 3% of patients who are searching right now – and Seth Turnoff argues that auction turns a clinical specialist into a commodity. “Go ahead and try to pay your overhead with clicks,” he says. “It doesn’t work.”

Seth Turnoff is Co-Founder and Managing Partner of Custom Medical Marketing (CMM), which helps independent specialty practices move away from relying on rented demand toward owning their own patient-acquisition channels. Through SEARCHLESS, CMM’s hypertargeted patient acquisition program, and the Branded Signal Methodology, a category-building framework for clinicians, he helps physicians differentiate and build lasting demand. He started in healthcare revenue cycle on a denials management team, fighting payers to get claims paid. He is a former touring drummer, a daily LinkedIn writer, and describes himself as contrarian by design. He is not a physician – he calls himself a “clinical tourist.”

Seth’s playbook is to skip the search auction and condition the 97% of future patients before they need care. Miguel’s limb-salvage practice went the other way: it dropped Google, Facebook and Instagram ads as noise and puts its money into liaisons who work referring providers, with self-referrals under 5%. Seth agrees that is the right call for limb salvage, yet warns that a referral-driven practice doesn’t control its own volume. Lucas keeps pressing the question every physician-owner asks: if nobody searched, how do you know the money worked?

“Being busy doesn’t always equal being chosen.” — Seth Turnoff

Why this matters: an independent practice can be full this quarter and still not own its demand. If your volume depends on a paid ad auction, a few referring doctors or a platform you rent, someone else decides how busy you are next year, not you. Seth’s central case is that the work of being chosen starts long before a patient searches, with a clear point of view and a brand that says something more specific, and more memorable, than a list of credentials. As Seth puts it in the episode, quiet clinical excellence only helps so many patients.

Who should listen: physician-owners deciding where to put their next marketing dollar, specialists who are tired of agencies selling clicks, and practices that live on referrals and want to know what happens when a referring doctor slows down. Seth speaks as a marketer who started in revenue cycle, fighting payers on denials, and Miguel and Lucas bring the practice owner’s view of what actually fills a clinic. If you have ever wondered if your marketing is working, this one is for you.

Key ideas from the conversation:
1. Credentials are table stakes; a brand is what patients and referrers remember you for.
2. Winning the search auction for the 3% can turn a specialist into a commodity.
3. The other 97% are future patients you can reach before they need care.
4. Referral-driven volume is real, but you do not control it.
5. Marketing without a sales back end, such as fast call-backs, wastes the spend.
6. Budget depends on the practice’s mode: startup, maintenance, growth or expansion.

• Why taking two days to call back a survey lead burns every marketing dollar on the front end
• How Seth decides a marketing budget by practice mode: startup, maintenance, growth or expansion
